Residential Services & Supports

Understanding the needs and desires of each person and providing the supports needed to help them live independently.

Lifetime Assistance recognizes that a safe and comfortable home is essential for achieving independence, dignity, and inclusive community living. Our goal is to enhance each person’s life by providing supports and services that meet individualized needs. A dedicated team works in partnership with each person and their circle of supports to create a person-centered plan that allows personal control over all aspects of their life and promotes increased independence.

Opportunities

Apartments

Apartments are located throughout the Rochester area and support services are provided based on each person’s needs. Direct Support Professionals (DSPs) are available from a few hours per week to twenty-four hours a day.

Homes

Lifetime Assistance offers a variety of different homes providing the supports necessary for people to achieve independence. Highly qualified DSPs provide training, guidance, and support at the level necessary to meet each person’s needs.

Perfect Fit/Family Care

Everyone deserves a chance to find their fit in life. For people with intellectual and developmental disabilities who live outside the family home, it can be challenging to achieve a true sense of belonging and personal fulfillment. Our Perfect Fit program provides a safe, natural, family setting for people to receive the care and support they need, with independence to explore their own interests. It offers a unique residential setting that welcomes people with warmth, compassion, and encouragement, enabling them to gain a genuine feeling of acceptance through stable relationships and community participation. Perfect Fit providers receive training and ongoing supports from Lifetime Assistance staff.

Community Living Opportunities

Community living options, for people of all abilities, is a quickly evolving landscape. There is a significant need for more residential opportunities better suited to the growing number of people with intellectual and developmental disabilities who are eager – and able – to live a more independent life.  These opportunities provide a foundation for success in their own homes and in their larger community.

Since 1978, Lifetime Assistance has been passionate about bringing hopes, dreams and goals to life. Whether it’s a supportive living home or apartment, a Perfect Fit arrangement, or in-home supports and services, Lifetime Assistance is here with the resources and expertise to help people gain independence and the quality of life they deserve – assisting family members and caregivers every step of the way.

Affordable Community Housing Opportunities

Frances Apartments

Located in Brockport, NY behind Lowe’s and close to Sweden Shopping Center, Brockport Plaza, Wal-Mart, and Brockport Corners Mall.

  • Frances I amenities include Energy Star appliances and fixtures, community room with laundry facilities and patio, resident computer lab, playground, central air, private entrance at ground level with patio or balcony on upper floors, private storage, paved off-street parking, and 24-hour maintenance. All apartments are located near a bus line. Water, sewer, and trash are included. Tenants are responsible for gas and electric bills.
  • 14 Units (twelve 1-bedroom units and two 2-bedroom units) are reserved for people with an intellectual/developmental disability who are receiving OPWDD Services.
  • Frances II amenities include Energy Star appliances and fixtures, washer and dryer hookups in each unit, community room with laundry facilities and patio, resident computer lab, playground, central air, private entrance at ground level with patio or balcony on upper floors, private storage, paved off-street parking, and 24-hour maintenance. All apartments are located near a bus line. Water, sewer, and trash are included. Tenants are responsible for gas and electric bills.
  • Eight units are reserved for persons with a Physical Disability/Traumatic Brain Injury.
